The Historical Society of Central Florida’s form provides a useful model for organizations updating or creating a new form.

This two page document is formatted with numerous check boxes that aid staff in assessing the criteria for deaccessioning, how an object should be disposed of, and a checklist for the proper documentation of the object. The form also provides space for assessing the condition, the identifying details on the object, and sign-offs for each step of the process from the appropriate staff members.
